The Intact Naga Skeleton is a random drop from the Hydra mobs in Nazjatar. Especially the elites drop it more often than normal monsters.
The item can be opened to receive a few Mana Pearls, however, the greatest reason to attempt and get it is, that the item is required for the achievement: Give Me the Biggest Bag You've Got.
