The Giant Crab Leg is a rare drop from all of the shelled monsters listed as 'Crabs'. However, do note that this item has an extremely low drop chance, below 1 per cent, with the exception of Sandclaw Stoneshell – a rare crab monster in Nazjatar.
While the item does not start a quest or give a clear explanation of what to do with it, your 'ally' will still take it. Who is it you ask? You can give it to Bladesman Inowari if you are with the Alliance or Poen Gillbrack, for the Horde players.
Turning the item in will give you 250 reputation with your basic Nazjatar faction and 50 experience for your followers.
It is important to note that since the item is marked as unique, you have to turn it in as soon as you get it. That is because no more Giant Crab Leg will drop for you as long as you have one in your possession.
