This item is part of the quest, Thazz'ril's Pick, in Durotar, in the mines, in the Valley of Trials.
The objective of the quest is finding the Thazz'ril's Pick and bringing it back.
To find it you have to enter the cave which can be found in the north of the area, then as you walk the mine you will find the splits in the path, you have to go straight on the first one, then go to the right, with a creek to your left, which you should follow, all the way until a waterfall. There it should be, since its somewhat illuminated, it should be fairly easy to find. Afterwards, just exit the mines and return the pick to the quest-giver.
