The skull itself is relatively easy to obtain, since all you need to do is kill Mor'Ladim, who should be around 19.2, 33.2 in Duskwood.
Do note, however, that he is an elite mob and as such it can be wise for some classes to either party-up for him, or to kite him some other way, since he hits quite hard and can be a bit of a hassle to defeat if you are not careful.
